Graduate School Forms

As you probably expect, you will need to fill out forms during your time in graduate school, but the number and details of the forms will depend on your degree program and the path you take to get your degree. This section provides general information about some of these forms, and is divided by type of degree program. For more information, go to the Graduate Records office in 150 Caldwell Hall (DE4) or check out the graduate school website at CLICK HERE. In addition, read through the Guide to Graduate Study (a small booklet put out by the Graduate School, www.gradschool.cornell.edu/pubs_and_forms/pubs/guidetograduatestudy.pdf) for printed documentation. Finally, you might also want to attend one of the sessions (held in October and June by the Graduate School Thesis Advisor) which discuss the forms and formatting needed for theses and dissertations.
